Description
We are excited to be attending Cloverleaf DPA this year on November 18, 2023 in Lyons, GA. Registration fee includes the child's participation fee, lunch fee, recreation and transportation for the day. It also includes two guest tickets for the parents\guardians of the student to attend. All students must complete this online registration or pay in person at the Extension Office by November 16th.

Requirements
Only current 4-H youth in this county should be registered for this event. If you are not from this county, please contact your local Extension office for more information regarding 4-H programming in your area.
After registration, the following forms are required for participation.
- The Georgia 4-H Medical Information and Release Form should be completed before this activity. This form allows you to share important information with us so we can provide the best care for your child.
- Should any medication be required during the activity, the Georgia 4-H Medicine Form and the medication should be given to the Extension chaperone upon arrival.
- The Georgia 4-H Code of Conduct establishes guidelines for expected behavior standards and consequences of misbehavior.
