Description
Join us for an afternoon of fun STEM activities & challenges.
Participants: Open to 4-H Cloverleaf-age students (grades 4–6)
Cost: $6
Paperwork (emailed to you once registered): All students, will be required to complete standard 4-H paperwork: enrollment form, medical release, and code of conduct.
4-H members will also be required to sign the liability waiver provided by The Learning Treehouse. This waiver states that The Learning Treehouse is not responsible while these students are on site for the 4-H STEM Club.
***The STEM Curriculum is designed for students in grades 4-6, so please register your child only if they are in this age group.***

Only current 4-H youth in this county should be registered for this event. If you are not from this county, please contact your local Extension office for more information regarding 4-H programming in your area.
After registration, the following forms are required for participation.
- The Georgia 4-H Medical Information and Release Form should be completed before this activity. This form allows you to share important information with us so we can provide the best care for your child.
- Should any medication be required during the activity, the Georgia 4-H Medicine Form and the medication should be given to the Extension chaperone upon arrival.
- The Georgia 4-H Code of Conduct establishes guidelines for expected behavior standards and consequences of misbehavior.

Refund Policy
The cost is $6 per club. If your child is signed up for a 4-H Afterschool Club and cannot attend for whatever reason you MUST notify the 4-H office no later than 24 hours in advance.Two no-call, no-show occurrences will result in your child’s inability to participate in any afterschool clubs for the remaining portion of the school year.
